Your first visit to Barkman and Smith Physical Therapy will start with a thorough assessment of your condition or injury and the development of a customized treatment plan.
It is possible that you will be receiving your first treatment at that time as well, so be sure to wear comfortable clothing.
Bring with you your photo identification, insurance cards and any test results from your referring physician.
If you normally wear corrective lenses or hearing aids, be sure to have them in place. Communication with your therapist is important.
Please bring a list of any prescription and over-the-counter medications you are currently taking.
